The PAY PERIOD begins on the first calendar day of the month and ends on the last calendar day of the month. Assuming all classes and lessons are held as scheduled, please project your total hours worked for the entire month.
If something changes between the time of your submission and the last day of the month, please notify directly.
You must respond to all items followed by an * with either a quantity or a 0 for none, no matter what class you teach.
DO include all lessons/classes you will have actually taught within the entire month, whether as an assigned teacher or as a sub.
DO NOT count classes that you have missed, whether due to cancelation or sub.
